Test Overview : To measure clozapine and metabolite in serum or plasma by liquid chromatography/ tandem mass spectrometry (LC-MS/MS) for evaluating toxicity and for monitoring therapeutic levels.
Test Preparation Instructions : Collect specimen prior to last dose or at least at the ¾ time. Mark off the dosing interval (i.e. Dose is every 24 hrs then collect at least 18 hrs after previous dose).

(1) CLOZAPINE + NORCLOZ

There are no standards indicating critical values for
serum clozapine/norclozapine levels. A review of the
literature indicates that only 2-4 anecdotal case reports
identified seizures at clozapine levels > 4000 nmol/L.
Although a relationship between dose at 500 - 600 mg/d and
seizures has been substantiated, there is no clear
relationship between doses and levels, and no evidence that
risk of seizures increases once dose has exceeded 500 -
600 mg/d. For help with interpretation of serum clozapine/

Reference ranges are valid only for appropriate collection
times: pre-dose (trough), just prior to the next scheduled
dose, or post dose at a time >75% of the dosing interval.

Collection Requirements: Specimen may be collected in Dr's office and submitted as per routine samples but must arrive at a Dynacare laboratory within 24 hours of collection. Samples will be frozen upon receipt in a Dynacare main lab facility. Centrifuge. Do not use separator gel. Separate serum or plasma from cells ASAP. Aliquot serum or plasma to a transport tube. Indicate date and time of last dose and date and time of specimen collection on both the transport tube and the requisition.
Storage and Transport: If specimen will be received at a Dynacare main lab location within 24 hours of collection, store and ship refrigerated. If not, store frozen and ship on dry ice.
